DESAFÍOS DURANTE LA EJECUCIÓN DE ENSAYOS CLÍNICOS
DE VACUNAS CONTRA EL VIRUS SARS-COV-2

Reciba un saludo cordial, en primer lugar, agradezco el tiempo otorgado para leer esta carta, redactada con la finalidad de
compartir con usted junto a los lectores de la revista, parte de mi experiencia al supervisar la ejecución de ensayos clínicos
de vacunas contra el virus SARS-CoV-2, partiendo de la amenaza que representa la pandemia de COVID-19 para la salud
pública, además de las implicaciones económicas, así como sociales a nivel global. En este sentido, frente a la ausencia de
un tratamiento, el rápido desarrollo de una vacuna segura y eficaz es considerada la manera más viable de controlar esta
pandemia. Pero en el trayecto hacia una solución, nos topamos con un primer obstáculo, en los últimos años las vacunas
hechas con tecnologías tradicionales han demostrado que no son capaces de brindar protección a la población mundial duran-
te la rápida propagación de epidemias emergentes. Por lo tanto, el establecimiento de enfoques novedosos que sean rentables,
capaces de ser desarrollados y producidos rápidamente a gran escala, son de suma importancia para la salud mundial (Brown
et al., 2020).
En consecuencia, las vacunas en estudio representan una alternativa prometedora con respecto a las convencionales, debido
a su elevada potencia, capacidad de desarrollo rápido, fabricación a bajo costo y administración segura. A pesar del avance
tecnológico, una vez que estas vacunas han alcanzado las fases de ensayos clínicos, nos hemos encontrado ante otros
desafíos, los cuales nombraré a continuación: (Brown et al., 2020). Es una carrera contra el tiempo, normalmente los ensayos
de las fases I, II y III se completan entre 2 a 5 años e incluso ocasionalmente más. Esto es necesario para demostrar que una
vacuna es segura, inmunogénica y eficaz. Sin embargo, debido a la magnitud de esta pandemia, los desarrolladores de
vacunas han tenido que realizar ensayos clínicos de Fase I y II paralelamente, para adquirir data lo antes posible y así poder
iniciar la fase III. (Pardi et al., 2020) Los ensayos de fase III constan de un extraordinario volumen de participantes nunca
antes visto, sumado a que deben ser reclutados y enrolados en un corto periodo de tiempo, aproximadamente de 8 a 10 sema-
nas, tomando en cuenta que debemos seguir protocolos de distanciamiento social además de medidas sanitarias en todo
momento, cuando los participantes acuden a nuestra clínica.(Khuroo et al., 2020).
La amplia diversidad racial en la población estudiada, para proporcionar la data requerida, necesita de una muestra de pacien-
tes que represente de manera generalizada los resultados de eficacia y seguridad. Por ende, se debe dar prioridad en la selec-
ción de pacientes que reflejen los datos demográficos de la actual pandemia, especialmente en comunidades vulnerables y de
alto riesgo como la población latina, negra e indígena. (Chastain et al., 2020) Se debe priorizar la inclusión de participantes
con historia de riesgo de exposición a COVID-19, para determinar que las vacunas son eficaces, debemos probar que funcio-
nan al administrarlas en poblaciones expuestas diariamente al virus. (Harrinson & Wu, 2020) Igualmente, gran parte de los
participantes deben tener co-morbilidades que los ponga en riesgo de desarrollar casos severos de infección por COVID-19,
pues debemos probar que la vacuna es eficaz al disminuir la severidad de la infección en estas poblaciones de alto riesgo
(hipertensión, diabetes, obesidad, enfermedades pulmonares crónicas, edad avanzada, entre otras dudas e incertidumbre por
parte de la población, especialmente en las comunidades más afectadas, estos desafíos van desde la desinformación promovi-
da por redes sociales hasta el rechazo de vacunas entre comunidades con mayor nivel educativo y la falta de confianza en las
agencias de salud pública.
Ahora bien, independientemente de los desafíos anteriormente expuestos, quiero tomar la oportunidad para felicitar a nuestro
equipo, pues han mantenido una actitud positiva y admirable, siempre con una gran sonrisa debajo de sus máscaras N95, en
