En esta investigación se analiza la efectividad de la GPC para pacientes hospitalizados con
choque hemorrágico traumático, en una institución hospitalaria de segundo nivel de atención, al
comparar un grupo de control histórico con un grupo de estudio.
Las características sociodemográficas y clínicas de la población en ambos periodos de estudio
fue similar, en correspondencia a lo reportado en artículos científicos ya que los hombres jóvenes
se exponen a conductas de riesgo, a la ingesta de alcohol, realizan deportes y trabajos de mayor
peligro, además el nivel de accidentabilidad, asociado a traumas contusos y a politraumatizado es
alto en el mundo y en Cuba. (Soler Vaillant R, (2021), Spahn D R, (2019), Caballero López A.(2020).
La literatura científica reporta que la HTA influye en el mal pronóstico de pacientes con choque
hemorrágico traumático, por constituir un importante factor de riesgo para otras enfermedades
crónicas y provocar daño endotelial a nivel de las grandes, pequeñas arterias y arteriolas, además
si estos pacientes llegan al hospital con cifras tensionales elevadas, el diagnóstico clínico del
choque se dificulta, presentan mayor sangrado y el control de la hemorragia se hace complejo. En
general las enfermedades crónicas definen un subgrupo de pacientes traumatizados con mayor
riesgo de muerte, por las graves complicaciones que se asocian a la hemorragia. (Labrada
Despaigne A, (2018), González Rey T, (2020).
Entre las complicaciones precoces se identificó la hipotermia que se encuentra en el 50 % de
los pacientes con choque hemorrágico traumático e influye en el 43 % de las muertes por
hemorragia, debido a que provoca graves complicaciones como el aumento de la resistencia
vascular sistémica, arritmias, disminución de la compliancia pulmonar, depresión del sistema
nervioso central, desviación hacia la izquierda de la curva de disociación de la hemoglobina,
coagulopatía aguda y acidosis metabólica. (Soler Vaillant R, (2021), Spahn D R, (2019), Caballero
López A, (2020), Stanworth S J, (2016), Hooper N, (2022).
Murad MK, (2012) al evaluar protocolos de tratamiento para pacientes con choque
hemorrágico traumático que incorporaron la prevención de la hipotermia, reportó una disminución
de la mortalidad del 44 % al 8 %, aspecto que no se pudo corroborar en la presente investigación.
En el tratamiento de los pacientes con choque hemorrágico traumático se utilizan diversos
métodos hemostáticos, pero es incuestionable que el éxito de los mismos va a depender del
estado de los componentes de la coagulación en cada paciente, por tanto la coagulopatía aguda
impide el adecuado control del sangrado y favorece la alteración del volumen sanguíneo y la
oxigenación tisular. Esta complicación está presente en el 25 % de los pacientes con choque
hemorrágico traumático y puede causar la muerte del 46 % de estos enfermos. (Soler Vaillant R, (
2021), Spahn D R, (2019), Caballero López A, (2020), Stanworth S J, (2016), Hooper N, (2022).
El principio de reanimación de control de daños se desarrolló a través de la observación de las
víctimas de guerra con hemorragias masivas, que eran transfundidas con proporciones
equilibradas de sangre y lograban corrección de la coagulopatía aguda con una disminución de la
mortalidad del 60 % al 19 %.(Eastridge BJ, 2019).
Se sugiere realizar la cirugía de control de daño en pacientes con choque hemorrágico
traumático, temperatura <35 ° C, lactato> 4 mmol/litros y un pH corregido < 7,3, se ha reportado
la supervivencia del 46 % de los pacientes.(Traynor MD, (2020), Gupta A, (2017).
Pino Sánchez FI, Ballesteros Sanz MA y Cordero Lorenzana L (2015), identificaron 1.572
indicadores de calidad para la atención del trauma, y los clasificaron en 8 categorías, entre las que
se encontró la adherencia a las guías de prácticas clínicas, la mayoría con poco soporte científico,
la única medida que mostró tener evidencia científica fiable y válida fue las muertes evitables, que
incluyó el tiempo de realizar cirugía urgente para el control definitivo de hemorragia y las muertes
por choque hemorrágico.
El promedio general de adherencia a las guías que reportó Rosas Ramos W, (2016) en su
investigación, fue de 71.87 % y el servicio de cirugía fue el que más se adhirió a las
recomendaciones, en un 95 %. Castrejón Arteaga JJ, (2022), demostró que el 60 % de los médicos
cumplieron con más de la mitad de las recomendaciones del manual ATLS y solo el 42 % se adhirió
a más de 3 recomendaciones de las guías europeas. El nivel de adherencia a la guía diseñada en
la presente investigación fue mayor a los reportes expuestos con anterioridad.
Varias investigaciones que analizan el funcionamiento de los sistemas de trauma enfatizan en
la importancia de minimizar el tiempo entre el ingreso y el control quirúrgico del sangrado de los
pacientes con choque hemorrágico traumático, por constituir un factor determinante en la
supervivencia de estos. (Soler Vaillant R, (2021), Spahn D R, 2019, Caballero López A, (2020),
Eastridge BJ, (2019), Henry S, (2018).
Estudios de reanimación de traumatismos en pacientes con hemorragia mostraron que la
mediana del tiempo hasta la muerte fue de 2,0 a 2,6 horas, el 50 % de todas las muertes ocurrieron
3 horas posteriores a la llegada al hospital.(Eastridge BJ, 2019).
Se sugiere que el reconocimiento temprano de la hemorragia y su control, se debe realizar en
una media de tiempo de 2 horas, (Cannon JW, 2018), parámetro que se cumplió en el presente
estudio.
Algunos artículos informan que hasta el 37 % de la mortalidad general en las primeras 24 horas
de los pacientes con trauma grave se atribuye a hemorragia aguda, por sistema de órganos, la tasa
de mortalidad observada por hemorragia en el trauma torácico es entre el 36 % y 67 %, en la región
abdominopélvica del 64 %, en la región axilar e inguinal del 61 % y la cervical del 39 %.(Eastridge
BJ, 2019).
En el presente estudio después de aplicada la GPC, se logró que la mortalidad precoz
disminuyera a un 12,3 %, por debajo de las cifras establecidas internacionalmente.
Se concluye que se cumplieron los propósitos de los indicadores de calidad previamente
asumidos en la investigación, por lo que se demostró la efectividad de la guía de práctica clínica
para el diagnóstico y tratamiento hospitalario de pacientes con choque hemorrágico traumático.
