Validation of the Cubillas Index for predicting lymph node involvement in inguinal lymphadenectomy for patients with penile cancer

Introduction: The Cubillas Index was developed to predict lymph node metastases in patients with penile cancer. It takes into account factors such as the histological grade of the tumor, the level of anatomical infiltration, and the presence of perineural infiltration.
Objective: This study aims to determine how reliable the Cubillas Index is as a tool for identifying lymph node involvement in penile cancer patients undergoing inguinal lymphadenectomy.
Methodology: A theoretical review was conducted, focusing on a specific research question and using the PICOT strategy along with DeCS/MeSH terms to search for scientific information in health databases such as PubMed, the European Association of Urology, and the American Association of Urology. Additionally, search engines like TripDatabase were used. Articles identified were evaluated using quality standards and the STROBE criteria. The selected papers spanned from 2010 to 2024 and had observational designs. Out of 282 articles identified, only 3 were selected.
Results: The results showed that the Cubillas Index was effective in predicting nodal involvement in patients with penile cancer undergoing inguinal lymphadenectomy. Factors such as higher histological grade (p<0.0001), depth of infiltration (p=0.0002), perineural invasion (p<0.0001), and vascular invasion were all significantly associated with nodal uptake. The area under the curve was 72% with a 95% confidence interval of 63–81%.
Conclusions: the Cubillas Index seems to be a reliable predictor of nodal metastasis in penile cancer patients undergoing inguinal lymphadenectomy
