ESTUDIO COMPARATIVO DE LA EFICACIA DE LA ADMINISTRACION DE LA ATROPINA MAS DIFENHIDRAMINA VERSUS ATROPINA EN INTOXICADOS POR INHIBIDORES DE LA ACETILCOLINESTERASA EN EL SERVICIO DE EMERGENCIAS DEL HOSPITAL PROVINCIAL DOCENTE AMBATO

Resumen

 Se trata de un ensayo clínico comparativo concurrente entre dos terapias: La administración de atropina a dosis/respuesta vs La administración de  atropina  más una sola dosis de difenhidramina.


Se determinó que la mayoría de los pacientes se encontraban entre 20-35 años edad, eran solteros y las circunstancias que determi- naron estos eventos eran intentos autolíticos, Una vez administradas las terapias  en base a atropina(1)  y atropina mas difenhidra- mina (2 ), en este tipo de pacientes se pudo comprobar que la ausencia de los síntomas con el esquema 1 fue más tardía en relación al esquema combinado (atropina mas difenhidramina) sobre todo en los pacientes intoxicados por organofosforados, no existiendo diferencias significativas en intoxicados por carbamatos con ninguno de los esquemas recibidos. En relación a la permanencia de los días de hospitalización fue mayor en los intoxicados por inhibidores de la colinesterasa con el Esquema 1 (atropina), en relación con el esquema 2 (atropina más difenhidramina), Asimismo pudimos evidenciar que con el tratamiento 1 (atropina) la colinesterasa séri- ca rebajada por efecto del tóxico mantiene niveles por debajo de lo normal, mientras que las colinesterasas muestran una diferencia significativa con tendencia a la mejoría cuando utilizamos el tratamiento 2 (atropina + difenhidramina).

 ABSTRACT:


 The  work is a concurrent comparative trial of two therapies: Administration of atropine dose / response vs The administration of atropine plus a single dose of diphenhydramine.


We could determine these results, most patients were between 20-35 years old, were single and the circumstances that led to these events were suicide attempts, once administered therapies based on atropine (1) and atropine more diphenhydramine (2) in these patients it was found that the absence of symptoms scheme 1 was more delayed in relation to combined treatment (atropine more diphenhydramine), especially in organophosphate poisoned patients, no differences poisoned by carbamates significant with any of the schemes received.Regarding the remaining days of hospitalization was higher in patients intoxicated by cholinesterase inhibi- tors with Scheme 1 (atropine), in relation to the scheme 2 (atropine more diphenhydramine), also show that treatment 1 (atropine) lowered serum cholinesterase by toxic effect of maintaining levels below normal, while cholinesterase show a significant difference with a tendency toward improvement when using treatment 2 (atropine + diphenhydramine).
