EFFECTIVENESS OF THE CLINICAL PRACTICE GUIDELINE FOR PATIENTS WITH TRAUMATIC HEMORRHAGIC SHOCK

Abstract

Severe trauma causes 5.8 million annual deaths, traumatic hemorrhagic shock causes half of these deaths. There are no studies in the province of Sancti Spíritus on the effectiveness analysis of the clinical practice guideline designed for patients with traumatic hemorrhagic shock.


The Objective was analyze the effectiveness of the clinical practice guideline for hospitalized patients with traumatic hemorrhagic shock.


An observational, analytical, cross-sectional study was carried out in patients admitted with traumatic hemorrhagic shock at the Hospital General Provincial Camilo Cienfuegos de Sancti Spíritus, during the years 2012 to 2019, to compare the results before and after the implementation of a guideline of clinical practice. The variables were grouped into sociodemographic, chronic diseases, injury mechanism, type of trauma, topographic location, early complications, medical-surgical treatment and early mortality and as indicators of effectiveness, adherence to the guideline, time between admission and start of definitive treatment and early mortality.


Male patients (84%), 60 years and younger (82.1%), hypertensive, (45%) with blunt trauma, (60.7%) polytraumatized, (47.5%) hypothermic prevailed, (38.4%) and with acute coagulopathy, (32%) and an early mortality of 12.3%. The purpose of the indicators was fulfilled.


The purposes of the quality indicators assumed in the research were fulfilled and the effectiveness of the clinical practice guideline for the diagnosis and hospital treatment of patients with traumatic hemorrhagic shock was demonstrated.
