EFFECTIVENESS OF THE CLINICAL PRACTICE GUIDELINE FOR PATIENTS WITH EXACERBATED CHRONIC OBSTRUCTIVE PULMONARY DISEASE

Abstract

Chronic obstructive pulmonary disease is a health problem and is the third cause of death in the world. Mortality is higher in patients who present exacerbations of this disease. The objective was evaluate the effectiveness of a clinical practice guideline in hospitalized patients with exacerbated chronic obstructive pulmonary disease.


A quasi-experimental analytical research was carried out, which included a before and after its implementation, at the Camilo Cienfuegos Provincial General Hospital in Sancti Spíritus, from 2017 to 2019. The variables were grouped into sociodemographic, clinical, severity classification in regarding the presence of acute respiratory failure, treatment, complications and as indicators of effectiveness hospital stay and mortality.


Female patients (57.2%) and age over 60 years (79.2%), serious and presence of life-threatening acute respiratory failure, (46.6%) received treatment with bronchodilators, steroids and antibiotics, (100%) antiviral, (50.8%) anticoagulants, (50%) protectors of the gastric mucosa (58.3%) and non-pharmacological treatment with oxygen (90.3%), mechanical ventilation non-invasive (28.2%) and invasive mechanical ventilation (8.7%). The purpose of the indicators was fulfilled.


The clinical practice guideline in patients with exacerbated chronic obstructive pulmonary disease applied was effective in complying with the proposed indicators.
