Average platelet volume as a clinical forecast predictor in septic states

Abstract

Introduction: The mean platelet volume is a biomarker of inflammatory, thrombotic and endothelial damage
processes that has become more important in recent years. Its provision in the daily clinical evaluation of
patients experiencing some septic condition is a cost-effective maneuver that can guide the physician to timely
decision-making, so it is important to investigate the relationship between VAW and sepsis, since its increase
is correlated with a poor prognosis.


Objective: To determine the direct relationship of the mean platelet volume (VPM) as a prognosis of mortality
in the first 72 hours in patients who undergo some type of sepsis diagnosed in the emergency service and who
subsequently enter the intensive care unit in a specialty hospital. in Quito.


Material and methods: Longitudinal and prospective cohort study that included patients over 18 years of age;
attended in the emergency, diagnosed with sepsis and who have been admitted to the intensive care unit were


included, from March 2016 to April 2017. The source of analysis was the digital longitudinal medical history
provided by the HOSVITAL computer system with measurements of confidentiality.


Results: 181 patients, 45.3% male and 54.7% female, aged between 40 and 95 years. The input VPM volume
was 9.8 fL and the upper range reached 10.35 fL. The mortality accounted for 48 hours after admission with a
VPM greater than 10 fL corresponded to 25% regardless of the place of hospital stay.


Conclusion: The implementation of the VPM in the daily clinical evaluation of patients with sepsis is an effective
tool that guides timely decision-making; follow-up and feedback of cases, in relation to therapeutic
behavior.
