Updates in the management of cardiogenic shock, a review of the literature

Abstract

Cardiogenic shock is a pathology that typically occurs after an acute myocardial infarction. Although there are more etiologies, AMI is the most common cause. This clinical entity continues to have a high degree of mortality because the patient can progress rapidly. having an imminent death, it is therefore necessary to analyze and study the management and treatment more widely. Objective: Describe the new updates in the management of cardiogenic shock. Methodology: This article is a bibliographic review that was based on 27 scientific articles obtained from information sources such as: Pubmed, Google scholar, Clarivate web of science and Scopus. The search strategy was based on the use of keywords such as. AMI, CHF, cardiogenic shock, management, treatment and ICP. Those studies that met the inclusion criteria were included, which are: having been written in Spanish and English, published in the last five years (2019-2023), that address the topic of cardiogenic shock and that they are original, accessible. complete and free. The exclusion criteria that were established are: articles that have been published before 2019, research that is not related to the topic of study and that have been written in a language other than English and Spanish. Results: Cardiogenic shock (CS) is more common in men aged 61 years, mainly caused by acute myocardial infarction (AMI). Hypertension and diabetes are common comorbidities. Percutaneous coronary intervention and the use of norepinephrine and dobutamine are essential. Mortality is 43.9% at 30 days. Discussion: It is difficult to establish a standard treatment for CS, due to its multiple causes and phenotypes. Prompt identification and proper management are crucial. Conclusions: CS, frequently caused by AMI, has high mortality, especially in older adult patients with hypertension. Advances in therapies and devices have improved treatment. Rapid detection and personalized management are essential to improve survival
