Abstract

Introduction: During pregnancy there are important anthomus-functional changes that have an impact on all
organic systems; one of the most affected is the renal system which must adapt to the new demands involved in the course of a pregnancy. Such adaptation is difficult when renal function is not adequate prior to the
period of review, determining an increased risk of adverse pregnancy outcomes, which are sought to decrease
through appropriate preconception management and during pregnancy Pregnancy. The coexistence of a
catastrophic pathology such as chronic kidney disease in need of haemodialysis, determines an increased risk
in the development and growth of a new living being, generating a gestation with high maternal and fetal
morbidity.

Objective: Describe a clinical case of chronic high blood pressure with over-lying preeclampsia in patients
with chronic kidney disease on haemodialysis.

Material and methods: Retrospective descriptive study, clinical case presentation.

Results: The clinical case of a 27-year-old female patient is present in her second trimester of gestation, with a
history of stage 5 chronic kidney disease in haemodialysis and secondary high blood pressure, who is
hospitalized for uterus inhibition due to threat of pre-term delivery, with high arterial stresses during
hospitalization and ultimately culminating in emergency caesarean section by over-successful preeclampsia
plus product loss.

Conclusions: The history of chronic kidney disease in dialytic need determines a high risk factor for the
development of hypertensive disorders in pregnancy, which despite proper management creates risk for the
mother and fetus.
